Through their seven regional teams, they support local integrated care systems (ICS) to improve the health of the population, improve the quality of care, tackle inequalities and deliver care more efficiently.
Challenges
Our client built a solution called NHS Pathways - a clinical decision support system used by the National Health Service (NHS) in the UK. It helps healthcare professionals, particularly those working in urgent care services like 111 call centres or out-of-hours GP services, to assess patient symptoms and determine the most appropriate course of action. The system is designed to guide clinicians through a structured process that can assess a patient's condition and recommend the correct level of care or medical intervention. The primary goal of NHS Pathways is to ensure patients receive timely and appropriate care, whether that means self-care advice, booking an appointment, or directing them to emergency services if necessary.
The system was originally an on-premises solution with a limited number of services, and the challenge was to re-platform and transform the service to contribute more to reducing pressure on front line clinical staff.
Mastek was engaged to re-platform, transform and run NHS Pathways as a scalable solution with the following objectives:
- Reduce cost
- Improve responsiveness
- Widen the usability and uptake of the Pathways service as a result of the improvements made
Mastek's Move
Mastek worked in a blended team with our client, providing the DevOps and Testing services as part of the project. Mastek also maintained the historical system during transition as part of our work.
Mastek used the following AWS integration components as part of our work on the Pathways DevOps program:
- S3 - For storing database backups, terraform state files and automated test reports
- RDS - For hosting SQL databases
- EC2 - For hosting web applications
- Secrets Manager - For storing secrets
- EKS - For hosting apps using Kubernetes
- ECR - For storing app images
- WAFv2 - For web application firewall
- Other - VPC, Route53, Lambda
The Impact
The NHS Pathways solution is now used by over 1 million citizens per annum.
The system is 75% more efficient and scalable post migration and transformation that has contributed to the growth in clinical pathways available and the uptake in use by citizens in the UK.
NHS Pathways telephone triage system is a clinical decision support system (CDSS) supporting the remote assessment of callers to urgent and emergency services and is now used in the following settings:
- NHS 111
- 999
- Integrated Urgent Care Clinical Assessment Services
- NHS 111 Online
- To assist in the management of patients presenting to urgent care or emergency departments
Use of these services by UK citizens has delivered significant cost savings to the UK Taxpayer.
